This agreement sets out the terms and conditions relating to our work together and is a requirement of my membership of the National Counselling Society.
1. You agree to the terms and conditions in these contract notes and accept full responsibility for receiving counselling.
2. We agree to work together to address the issues presented by you, however, because of the individual nature of the treatment, and differing psychological makeup of individuals, no guarantee of a result or cure can be given.
3. Each appointment lasts for approximately 50 minutes balance is to be paid at the end of the session (please see Cost of Sessions Page for various charges). A non- refundable £20 deposit is required to secure you appointment and the balance is to be paid at the end of the session. Please provide at least 48 hours notice if you need to change your appointment day/time.
4. ‘Consultation’ related conversations between our sessions will, where possible, be limited to text or email.
5. Your welfare is my primary concern and will only take second place if not to do so would seriously jeopardise other members of the public or my welfare.
6. I will ensure that your confidentiality will be maintained in all, but the most exceptional circumstances and all information collected during the sessions will be protected at all times. Information will only be disclosed under a Court Order (civil, criminal or coroner’s Court) or where not to disclose could potentially, in my professional opinion, cause danger or serious harm to yourself or others.
7. There may be slight risks associated with psychological services. During the process of treatment, there may be reason to review some unpleasant memories or to experience some uncomfortable emotions. It is acknowledged that making personal changes in behaviour, thinking and emotions through counselling sometimes requires learning by trial and error and some confusion or setbacks in the process may occur. You acknowledge and agree to accept these potential risks.
8. As part of my ongoing supervision I may share verbal details supervisor in order to ensure I am providing the best service possible. All information will be anonymous, and this will not be a breach of professional confidentiality.
9. Remote consultations via Zoom are available should circumstances necessitate this but clients are reminded of the potential security implications of such services and take full responsibility for them.
10. I consent to details of my medical and treatment records being kept safely and securely to meet current GDPR rules.
